As categories in conjunction with the process to constitute illness narrative of the mental disorder,[The staff who was chosen by understanding the patient], [Having felt ability and growth of the patient that came to be able to make a relationship with other people],[Seeing through the ability that the patient can look back on own severe past] were extracted.
When the patient announced their experience of illness, they expressed own pride and future hope wealthily. The charge staff and the patient experienced difficulty to constitute announcement contents of the experience of illness in a short time and felt a hesitation to speak serious experience, but they were able to be satisfied after the announcement, and the staff detected the ability and the growth of patient that came to be able to make a relationship with other people. And the staff felt the joy that the fragmentary story of the patient was able to understand as a series of stories for the first time.

Research on Nurses" Consciousness and Skills for Protecting the Human Rights of People with Mental Disorders

1. We interviewed 28 psychiatric nurses with the aim of establishing what ethical problems they experience, what conflicts in values occur. As a result of qualitative analysis, ethical problems experienced by the subjects were allocated into 8 categories: "Patient's right", "Treatment", "Discharge and long term hospitalization", "Transfer and ambulance admittance", "Condition of patient"," Hospital ward rules", "Persons" and "Care. "The factors of values included in these problems were allocated into those relating to "patient rights", "treatment values and nursing professional values ", "hospital values as place and function", "actual limitations", "personal values", "cultural values" and "rights of persons other than the patient". We confirmed that psychiatric nurses experienced ethical conflicts between these values in the field.
2. We developed an 80 item questionnaire, regarding the Frequency of Experiencing Ethical Problems (FEEP), based on our qualitative study. Subjects were nurses working in psychiatric hospitals chosen by proportional stratified random sampling with the aim of reflecting the ratio of numbers of psychiatric nurses in public institutions to ones in private institutions. We obtained 882 responses (88.6%) in 28 psychiatric hospitals. The problems most frequently experienced by nurses were difficulties regarding patient discharge: "difficulties discharging patients because of aging family member", "difficulties discharging patients because of the lack of social resources", etc. The total FEEP score was significantly correlated with the distinction of nurses' licenses (RN>LPN), whether or not the ward could admit coerced patients, whether or not the ward could admit patients during holidays or night time and the openness of the wards. Factor analysis identified that FEEP comprised six factors: "Work environment", "Coerced treatment", "Nurse's responsibility and feeling", "Team treatment", "Discharge and self-determination" and "Informing about diagnosis".
3. We surveyed the participants in the lectures about psychiatric nursing ethics and confirmed the necessity of developing the educational program suited to the field.

DEVELOPMENT OF NURSING CARE MODEL FOR DRUG DEPENDENT PATIENTS

At the first stage of this study, through the field work in the special word for drug dependent patients and interviewing 16 nurses in charge of caring for drug dependent patients from 5 facilities, we developed a tentative nursing model for drug dependent patients based on the clinical situation in Japan, which is consisted of contents of caring according to the specific 3 stages (withdrawal stage, craving stage, maintenance stage) and nurse's feeling on caring for drug dependent patients.
At the second stage of the study, we developed an original questionnaire being based on the tentative nursing model given in the first stage of this study, and investigated 430 nurses about their nursing practice for drug dependent patients.
As results of statistical analysis, the following matters were revealed.
1. As contents of caring for drug dependent patients, the following 5 factors were identified.
(1) Respecting patient's own self, (2) dealing with troubles, (3) having relation with empathy, (4) having consistent relation, (5) rehabilitation support.
2. As nurses' troubles on caring for drug dependent patients, the following 4 factors were identified. (1) anti-social action of patients, (2) difficulties particular to the nursing of drug dependent patients, (3) patient's symptoms and social context, (4) the problems of nursing system. A significant relationship was found between "patient's symptoms and social context" and "the degree of trying to care".
3. A relatively strong correlation was found between the total score of "nurses' professional life worth living" and "the degree of trying to care".
4. As the feeling of nurses on caring for drug dependent patients, "anger", "guilty feeling" and "empathy" were founded.
Integrating these results, a nursing model for drug dependent patients was demonstrated. These results suggested the importance of special education of nursing for drug dependent patients in both of under graduate program and continuing education program.
