[Purpose] The educational effects of an on campus alternative training program conducted for first-year students in FY2022 which replaced clinical early exposure training, due to the spread of COVID-19 infection, were verified and compared with the results of FY2021, previously reported in our first report. [Participants and Methods] The study targeted first-year students in a physical therapy department in the 2022 academic year. Alternative training consisted of guidance from external instructors and school teachers. A new communication training using the world cafe method, which was not conducted in the 2021 study, was introduced, and learning achievement was investigated by questionnaire and compared with the results of the 2021 study. [Results] The simple 7-point Likert Scale showed that all items received positive responses. In the tally of frequently used words, “communication” was selected more often in 2022. [Purpose] Due to the spread of the COVID-19 infection, off-campus clinical early exposure training became impossible. The educational effect of alternative training on-campus as an emergency measure was investigated. [Participants and Methods] The subjects were 102 first-year Department of Physical Therapy students and 8 external teachers. A questionnaire survey of the subjective learning achievement and learning content was performed. [Results] The subjective learning achievement in the affective, cognitive, and psychomotor domains were all answered positively. Experience gained was in the whole affective domain, rehabilitation in various fields, the way of communication, symptom and physical therapy of patients through videos. The experience not gained was communication with real patients, the tension and atmosphere of the medical workplace, and details of patients' condition. [Conclusion] It is important to improve the quality of on-campus education for clinical practice. - Questionnaire survey of sport-related injuries in sport climbers

[Purpose] The maintenance effects of proprioceptive neuromuscular facilitation (PNF) special techniques were investigated using the reaction time (RT). [Subjects] The subjects were 40 healthy adult males. [Methods] The subjects were randomly divided into 4 groups of 10 persons, 3 groups of PNF techniques, joint exercises classified according to changes in speed, of initial stretch (IS), stabilizing reversal (StR), and combination of isotonics (CI), and an elbow flexion group, which executed the respective movements. To investigate the maintenance effect, RT was measured 7 times, immediately before the interventions, and at 0, 5, 10, 15, 20 and 30 min afterwards. [Results] In the IS and StR groups, RT was found to be shorter than before the intervention, until 10 min after the intervention, and in the CI group, it was shorter than before, the intervention until 15 min after the intervention. In the elbow flexion group, no significant differences from pre-intervention were found in the post-intervention RT. [Conclusion] The compound movement groups showed post-intervention maintenance effects lasting for 10~15 min, suggesting the utility of compound movements as physical therapy techniques in clinical applications. - Short-term Effects of PNF according to Types of Joint Movement

[Purpose] We examined the short-term effects of two types of stretch utilized in the specific techniques of proprioceptive neuromuscular facilitation (PNF). [Subjects] The subjects were 20 healthy adult males. [Method] We used two techniques categorized under “repeated stretch” from among the PNF specific techniques. Before and after PNF, we measured the reaction time (RT) of elbow flexion, and examined the maintenance of its effect. [Results] In the Repeated Stretch group, we found a shortening of RT, compared to before PNF, which lasted for 20 min from immediately after PNF. In the Slow Reversal group, we found a significant effect only at immediately after PNF. [Conclusion] Since the shortening of RT was maintained for 20 min in the Repeated Stretch group, it is possible to apply this intervention in the clinical application of physical therapy techniques. - Duration of Short-term Effects of PNF According to Different Types of Joint Movement

[Purpose] Utilizing the reaction time (RT), we investigated the short-term effects of proprioceptive neuromuscular facilitation (PNF) specific techniques, employing 8 kinds of stretches. [Subjects] The subjects were 40 healthy adult males. [Methods] From among the specific techniques, we employed four kinds of unilateral joint movements (simple movement), and investigated the duration of the effect, by measuring elbow flexion before and at specific time points after PNF. [Results] We found an RT shortening effect, compared to pre-treatment, which lasted for 5 min at the most among the groups receiving the specific techniques of rhythmic stabilization, contract-relax and hold-relax; no significant differences were found for the rhythmic initiation group. [Conclusion] PNF applications in physical therapy interventions are possible, since RT was shortened for 5 min after simple movements. - Short-term Effects of PNF in the Sitting and Supine Positions

[Purpose] We investigated the short-term effects arising from basic techniques of proprioceptive neuromuscular facilitation (PNF) using the response time. [Subjects] The subjects were 40 healthy men. [Method] In the sitting and supine positions, we measured the times of elbow flexion movements in the Resting (R) group, Elbow Movement (EM) group and PNF group, which received a PNF intervention of flexion and internal and external rotation, and investigated the duration of effect using the response time (RT) immediately before, just after and at 5, 10, 15, 20 and 30 min after intervention. [Results] In the PNF group, we found that RT was shortened compared to before intervention for 20 min after the intervention in both the sitting and supine positions. In the R and EM groups no significant differences were found. [Conclusion] Irrespective of posture, a short-term PNF effect lasting for 20 min was found in the the PNF group, and we consider this useful for the practical application of physical therapy techniques. - Continuous Effects of PNF-Focusing on Reaction Time-

[Purpose] The purpose of this study was to investigate the duration effect of PNF pattern, from the reaction time (RT). We divided the subjects into a PNF group, with flexion-adduction-external rotation of the upper extremity, and a non-PNF group without PNF. We measured the RT of elbow flexion before and after the exercise and over a period of time, to investigate the duration effect. [Subjects] The Subjects were 20 healthy males. [Methods] We divided the subjects into a PNF group, who underwent a flexion-adduction-external rotation pattern of the upper extremity, and a non-PNF group who were told just to flex and extend their elbow. We measured the RT during elbow flexion movement before exercise, right after exercise, and at 5, 10, 15, 20, and 30 minutes after exercise. [Results] There were no significant differences among the results of the non-PNF group. For the PNF group, there were significant differences in RT between pre-and post-exercise, and 5-20 minutes after exercise. [Conclusion] The results suggest that arousal of the central nervous system was maintained for up to 20 minutes by PNF.

At the request of the Japan Alpine Club, I accompanied former world champion and professional climber Yuji Hirayama as his trainer. He was the first person in the world to complete the El Niño route on the giant wall of Yosemite National Park.
