BACKGROUND: The effect of trunk stability and dynamic balance warm-up exercises on physical functional improvement remains unelucidated. This study examined whether exercises could prevent anterior cruciate ligament (ACL) injury and improve trunk muscle activation and dynamic balance in gymnasts. METHODS: This comparison study, involving gymnastics practice sessions, included 31 university gymnasts and was conducted in two periods: 1 year of observation followed by 2 years of intervention. Participants performed a trunk and dynamic balance warm-up exercise program during the intervention. The effect of exercise on the incidence of ACL injury was evaluated. In addition, the paired t-test was used to compare the Y-balance distance and the changes in muscle thickness associated with trunk muscle activation at rest and during plank. RESULTS: ACL injury risk during the intervention was significantly lower, with a relative risk of 0.23 (P=0.02, 95% CI: 0.06-0.88). Changes in muscle thickness with activation of the transversus abdominis (P<0.01, mean difference 4.1, 95% CI: 9.97-28.07, Cohen's d=0.52), internal oblique (P<0.01, mean difference 5.2, 95% CI: 9.72-21.55, Cohen's d=0.65), and external oblique (P<0.01, mean difference 5.5, 95% CI: 20.44-39.09, Cohen's d=0.71) muscles were significantly higher during the intervention. The Y-balance distance was also significantly greater in the posterior medial reach (P<0.01, mean difference 3.3, 95% CI: 1.56-6.26, Cohen's d=0.46) during the intervention. CONCLUSIONS: Exercise-based warm-up programs may decrease ACL injuries. It can improve physical functions, such as the rate of change in trunk muscle thickness and the posterior medial distance during Y balance. - Throwing kinematics and elbow varus torque relative to ball size in junior baseball players.

[Purpose] The purpose of this study was to investigate changes in the shoulder and elbow joint angles, upper limb angular velocities, and elbow varus torque when throwing balls of two different sizes. [Participants and Methods] The pitching motion of 26 junior baseball players was analyzed using an optical motion capture system. The balls used were a standard baseball and a small ball of equal weight. Shoulder external rotation/abduction and elbow flexion were measured. The maximum values of shoulder joint internal rotation, elbow joint extension, wrist flexion angular velocity, and elbow joint varus torque were also evaluated. The ball velocity was determined as an index of pitching performance. [Results] The shoulder external rotation and elbow flexion angles were higher when pitching with a small ball. The joint angular velocity was also significantly higher when pitching with a small ball for all items examined. The ball speed was significantly higher with the small ball. The maximum varus torque of the elbow joint divided by the ball velocity was significantly lower for the small ball. [Conclusion] For a junior baseball player with a small hand length, using a small ball enables pitching with low stress on the elbow joint. - Influence of pitching grip on shoulder and elbow injuries in junior baseball players, focusing on hand length and the metacarpophalangeal joint angle.

[Purpose] We investigated the relationship of pitching grip with hand length and index finger metacarpophalangeal joint angle. [Participants and Methods] A total of 650 junior baseball players divided into two groups according to whether they experienced shoulder or elbow pain were divided into two groups based on their pitching grip, i.e., those who held the ball with the ulnar side of the thumb and those who held it with the finger pad of the thumb. The metacarpophalangeal joint angle was measured while the participants held a ball. [Results] The average hand length was 16.3 ± 1.4 cm for those holding the ball with the ulnar side of the thumb and 15.8 ± 1.2 cm for those holding it with the finger pad of the thumb. Hand length and metacarpophalangeal angle were significantly greater in the group using the ulnar side of the thumb. The two groups showed no significant differences in the distribution of grip type or history of shoulder/elbow pain. The index finger metacarpophalangeal angle and shoulder/elbow pain were significantly lower in the pain group. [Conclusion] With respect to the pitching grip for junior baseball players, we recommend a slightly flexed rather than extended position with the metacarpophalangeal angle. - Muscle activity during bridge exercises on different types of floor surfaces
