  • Effectiveness of Simulation-based Education for Nursing Students Using a High-Performance Simulator

    Abstract
    Background: Simulation-based education for healthcare professionals began in the 1960s and was
    introduced to Japan in the late 1980s. At University A, pediatric nursing practicum is conducted using
    SimJunior, a high-fidelity simulator.
    Aim: This study aimed to investigate the effectiveness of a case study practical exercise using SimJunior.
    Methodology: The survey was conducted using a self-administered questionnaire. This study involved
    129 second-year nursing students from University A. The findings from the exercise using SimJunior,
    briefing 1 week before the practical exercise, and debriefing were studied and analyzed.
    Results: The practical exercise conducted using SimJunior confirmed that students gained a better
    understanding of respiratory status through visualization. The briefing held 1 week before the practical
    exercise helped students review their knowledge and understand the case studies and contributed to
    improving their observation skills. In addition, the debriefing facilitated the exchange of ideas and the
    retention of knowledge and skills among students.
    Conclusions: The practical exercises in the case studies using SimJunior were very effective. The
    simulator allowed the students to refresh their skills and knowledge .The briefing held 1 week before the
    practical exercise provided prior learning to students in assessing the condition of the pediatric patient
    and reviewing their knowledge. The exchange of opinions among students during the briefing led to the
    retention of knowledge and skills.

  • Problems Faced by Mothers of whose Children have Survived Cancer that Appeared During the Life Stages of the Children and the Process and Guidance for Self-Care

    Background: Childhood cancers affect 2,500-3,000 children each year, and there are approximately 50,000 childhood cancer survivors who have completed therapy for their childhood cancer. The onset of childhood cancer most commonly occurs during the period of infancy to early childhood, and unlike adult-onset cancer, people who have cancer as children are expected to encounter many problems throughout their life. This study aimed to identify the kinds of problems that are faced by childhood cancer survivors on reaching adult age and the kinds of self-care that they perform. Methods: Semi-structured interviews were conducted with nine adult childhood cancer survivors following an interview guide prepared in advance. The data obtained was analyzed using the modified grounded theory approach. Subjects were explained about the study prior to obtaining their consent. Results: The analysis of their problems and self-care resulted in the development of four core categories, eight subcategories, and 39 concepts. The core categories were acceptance of and self-care for sequelae, secondary cancers, and late complications, Problematic situations during their growth and development, proactively obtaining information, and acceptance and appreciation of their own life. Conclusion: Childhood cancer survivors were suffering from sequelae resulting from their treatments for long periods. Although they faced many other issues, they did not talk about them to their mothers or medical staff, leaving them unresolved. Furthermore, these issues evolved as the survivors grew. The amount of information for those who have experienced childhood cancer was small, and effective self-care was not achieved.
  • Rethinking Support for Pediatric Cancer Survivors Provided by Pediatric Specialty Hospital Outpatient Nurses

    Background: In Japan, the number of pediatric cancer cases was approximately 2,200 in 2019. Notably, 15 hospitals are designated for pediatric cancer in Japan. This study focused on the outpatient nurses of a pediatric specialty hospital supporting these designated hospitals. The study aimed to clarify the support that these nurses provide to pediatric cancer survivors as well as their thoughts on supporting the patients at pediatric specialty hospitals. Methods: This study used qualitative inductive research methods and conducted semi-structured freeresponse interviews with outpatient nurses working at a pediatric specialty hospital. Data analysis was performed using the Modified Grounded Theory Approach (M-GTA). Results: Overall, 15 outpatient nurses were included in the study. Moreover, the M-GTA analysis generated four core categories. The thought process related to a long-term follow-up outpatient clinic for pediatric cancer survivors included considering factors that posed difficulty in the establishment of such a clinic, reconsidering the importance of establishing such a clinic, and contemplating about establishing a desirable clinic. This was based on the belief of the outpatient nurses regarding building a trusting relationship with the patients. Conclusion: Owing to manpower issues, no guidance was provided for the pediatric cancer survivors at outpatient clinics, and follow-up care was provided only by physicians. Moreover, outpatient nurses recognized the problems faced by patients throughout their lives and understood the need to solve them, but they could not provide effective guidance. Because pediatric cancer survivors are at high risk of disease- and treatment-related outcomes, there is a need to develop a system that facilitates collaboration between different professional groups in order to improve their quality of life.

  • Mother's thought on the status of announcement to children with childhood cancer who underwent surgery in infancy and the response of the infant

    小児がん経験者の説明に対する母親の思いについて、インタビューガイドを用いた半構成的面接調査を実施した。対象者は子どもが乳児期に小児固形悪性腫瘍で手術を行った母親2名であり、インタビュー時の子どもの年齢は共に10代後半であった。録音した面接内容から逐語録を作成し、データとした。疾患の説明については、それぞれの両親の判断で母親が行っていた。それぞれの母親が行った説明方法は様々であり、そこには親の意向が強く表れていた。疾患の説明を母親が一方的に行った対象者と子どもの成長に合わせ、段階的に行った対象者では、どの後の親子関係の影響に差があると考えられた。どちらの親子も医療機関から離れた後は、手探りで子どもとの関係を構築しており、関係性が困難と思われる事例には、医療者が早期に介入することで修正が可能と考えられた事例もあった。完治した小児がん経験者にも定期的な医療者の介入を行うなどの必要性があることが示唆された。

  • 初年度OSCEにおける学生の到達度評価と今後の課題

    平成27年度に初めて実施した看護実践能力を測定する客観的臨床試験(OSCE)の課題ごとの特典傾向と特徴から学生の到達状況を把握し、OSCE後の学生への質問紙調査の結果より、学生への導入、課題への妥当、評価、模擬患者の導入等などのOSCE全般の成果と問題点について明らかにした。対象となった学生は、平成27年度にOSCEを受験した4年生103名のうち、研究対象者になることの受諾書を提出した99名であった。各課題の平均得点は、成人が最も高く(15.5)、小児(15.1)、高齢者(14.5)、精神(13.6)であった。各課題の平均得点率では、いずれの課題も概ね70%以上を習得しているという結果であった。課題の評価項目はできるだけ具体的なレベルで表現することで学生には実践する内容が明確になり、評価者にとっては評価ポイントがつかみやすくなると考えられた。課題について難易度の違いが見られ、今後、適正な課題の設定について検討する必要性があることが示唆された。

  • Influence of School Organizational Systems for Child Abuse on Elementary School Yogo Teachers' Responses to Cases of Abuse

    With the aim of determining appropriate ways for elementary school yogo teachers to respond to child abuse, this study examined school organizational systems for child abuse with regard to their influence on how yogo teachers suspect and identify cases of child abuse and collaborate with external parties or organizations. Self-report questionnaires on school organizational system for child abuse and collaboration with external parties were distributed to every yogo teacher in all 344 elementary schools in Prefecture A. Questionnaires were collected from 146 yogo teachers (response rate 42.2%). Findings suggested that the existence of a school organizational system to handle child abuse was related to early detection of child abuse. The reason for not collaborating with external parties even when child abuse was suspected or identified was that the teachers either did not feel certain the abuse was happening or were concerned about the outcome of consulting or reporting the situation to external parties. This suggests yogo teachers lack of knowledge and information about the external parties involved in handling child abuse. To prevent delay in detecting abuse, future studies should explore factors related to difficulties in establishing necessary organizational systems, and examine how each discipline and involved party can better understand each other's roles and specifically how they can effectively collaborate with each other.
  • Current situation and issues relating to child healthcare activities of nurses at day care centers due to differences in work allocation

    The aims of this study were to examine the current responsibilities of nurses in child healthcare activities conducted at day care centers, clarify the relationship between their work responsibilities and style of work allocation at these centers, and discuss the issues relating to the work allocation of nurses at these centers for the future. Research was carried out with 41 nurses who participated in a workshop run by the childcare council in Prefecture A, out of a total of 71 nurses working in day care centers in the prefecture. The nurses responded to a questionnaire created by the authors. The 30 questionnaire items concerning the allocation of healthcare activities in the day care centers were divided into three aspects: support for children, support for families, and cooperation with various professionals and related agencies. Differences in the three aspects were compared between nurses responsible for particular classes and nurses not tied to responsibilities for particular classes. The results revealed that mean scores were higher for the nurses not tied to responsibilities of particular classes in all three aspects examined. This indicates that in order for nurses at day care centers to make best use of their specialized skills and knowledge, and carry out child healthcare activities, the styles of work allocation must be improved.

  • 小児がん患児の家族支援に関する研究の動向と課題

    小児癌患児の家族支援に関する研究の動向を明らかにすることを目的に、1998年〜2008年の論文を対象に医学中央雑誌Web版を用いて検索を行い、得られた62件を分析した。その結果、研究対象患児は幼児と学童期が多く56.5%を占め、思春期・青年期は11.3%であった。支援対象の家族は、母50%、両親37.2%、父3.2%で、同胞を含めた家族全体を捉えた研究は3.2%であった。また、研究内容は、【治療や闘病の中で生じる問題に対する家族の不安や思い】【ターミナル期・死別後の家族の思いや支援】【癌の発症による家族関係の変化とソーシャルサポート】【初期治療後の患児・家族に対する身体的・精神的・教育的支援】【癌告知に対する親の苦悩と支援】の5つのカテゴリーに分類できた。
