  • Investigating optimal drainage cannula for venovenous extracorporeal membrane oxygenation: impact of side holes on blood oxygenation – an in vitro study

    Venovenous extracorporeal membrane oxygenation (VV ECMO) relies on effective blood drainage, and cannula design plays a crucial role in optimizing blood oxygenation. Recirculation, where oxygenated blood returns to the drainage cannula without systemic circulation, can negatively impact ECMO efficacy. It was hypothesized that drainage cannulae with a longer side hole portion would reduce recirculation and improve blood oxygenation. This in vitro study investigated the impact of the length of the side hole portion and the intervals of the side holes on blood oxygenation in a simulated VV ECMO circuit. Five cannulae were prepared with the following side holes: (a) 1-cm intervals from the tip to 5 cm; (b) 1-cm intervals from the tip to 10 cm; (c) 1-cm intervals from the tip to 15 cm; (d) 3-cm intervals from the tip to 15 cm; and e) 5-cm intervals from the tip to 15 cm. With blood drained from the right atrium and returned to the superior vena cava, recirculation rates and PaO2 at 150 s after ECMO initiation (descending PaO2 order) were: (c) 32 ± 1% and 160 ± 4 mmHg; (d) 35 ± 1% and 149 ± 5 mmHg; (b) 38 ± 1% and 139 ± 5 mmHg; (e) 39 ± 1% and 133 ± 4 mmHg; and (a) 57 ± 3% and 94 ± 7 mmHg. These findings suggest that cannulae with a longer side hole portion may optimize blood oxygenation by reducing recirculation. Furthermore, shorter side hole portions on drainage cannulae are not recommended for VV ECMO.

    INTRODUCTION: There is no standard for insoluble particulate matters in dialysate. We evaluated the insoluble particulate matters in online prepared substitution fluid. METHODS: We evaluated particle diameters and constituent elements of particles in online prepared substitution fluid and the final endotoxin retentive filter (ETRF). RESULTS: The measurement results with particles in the online prepared substitution fluid and particles attached to the final ETRF revealed that Ca-containing particles accounted for 11 particles and Si-containing particles accounted for 19 particles of 30 particles detected in the online prepared substitution fluid and the final ETRF. CONCLUSION: We presume that insoluble particulates in online prepared substitution fluid were Ca and Mg precipitated from dialysate and Si precipitated from dialysis water. Even if two ETRFs were connected in series, these particles were formed in the final ETRF.

    INTRODUCTION:: Recently, the use of veno-venous extracorporeal membrane oxygenation for adult patients with severe acute respiratory failure has increased. We previously investigated the optimal return cannula position; however, the optimal drainage cannula position has not yet been fully clarified. The aim of this study was to investigate the optimal drainage cannula position. METHODS:: Veno-venous extracorporeal membrane oxygenation was performed in four adult goats (mean body weight 59.6 ± 0.6 kg). The position of the drainage cannula was varied among the right atrium, the upper inferior vena cava, and the lower inferior vena cava, whereas the position of the return cannula was fixed in the superior vena cava. The recirculation fraction and arterial oxygen saturation and pressure (SaO2, PaO2) were measured in all drainage cannula positions. RESULTS:: In the lower inferior vena cava drainage cannula position, the recirculation fraction was the lowest. In the lower inferior vena cava, upper inferior vena cava, and right atrium drainage cannula positions at 3 L/min, SaO2 and PaO2 after 20 min were 92.9% ± 4.9% and 75.1 ± 26.0 mm Hg, 99.5% ± 0.5% and 113.8 ± 20.9 mm Hg, and 93.8% ± 6.2% and 91.9 ± 17.7 mm Hg, respectively. CONCLUSION:: With respect to blood oxygenation, the optimal position for the drainage cannula was the upper inferior vena cava. These findings suggested that blood from the superior vena cava, inferior vena cava, and hepatic vein was most efficiently drained in the upper inferior vena cava cannula position.

    The number of cases of veno-venous extracorporeal membrane oxygenation (VV ECMO) using the Avalon double-lumen catheter (Avalon-DLC) has been increasing. The catheter has many merits, such as less bleeding and reduced risk of infection. However, the diameters of the drainage and return lumens of the Avalon-DLC are smaller than those of cannulae for dual cannulation. Therefore, sufficient bypass flow may not be achieved, and blood oxygenation may be insufficient. The purpose of this study was to investigate whether VV ECMO using an Avalon-DLC can provide sufficient gas exchange performances.

    An Avalon-DLC was inserted into eight adult goats (body weight 60.2±0.4 kg), and VV ECMO was performed. The arterial oxygen saturation (SaO2), arterial oxygen partial pressure (PaO2), and arterial partial pressure of carbon dioxide (PaCO2) were measured under only ECMO support without native respiration.

    When the bypass flow rates were higher than 2L/min, the SaO2 was maintained at greater than 80%, and the PaCO2 was approximately within the normal range. At 20 minutes after the support, PaO2 was 23.7±1.2, 46.3±3.2, 91.9±9.7, and 159.6±29.8mmHg at 1, 2, 3, and 4L/min, respectively.

    VV ECMO using the Avalon-DLC could obtain sufficient gas exchange.
