- History of Judo Therapy -From the Dawn of Judo therapy to the Reiwa Era-

Judo therapy, a unique traditional Japanese medical treatment, has profoundly evolved with Japan's societal and medical landscape since ancient times. Its roots trace back to the Asuka and Nara periods, when Chinese Seikotsujutsu (orthopedic techniques) were introduced and the martial arts concept of Katsuho (healing injuries) began to emerge. Early foundational knowledge is even found in the ancient Ishinpo medical text. The Muromachi and Sengoku periods were pivotal, as Katsuho saw significant practical advancements amidst constant warfare. Martial artists refined reduction and fixation methods for wounded soldiers, solidifying these as secret teachings within their schools. During the Edo period, Katsuho transitioned from martial application to public care, as "Honetsugi-isha" (bonesetters) widely treated the general population. Techniques were systematized and refined, and knowledge was formally disseminated, establishing the profession's foundation. The Meiji era brought a crisis for Judo therapy with the introduction of Western medicine. However, its value was reaffirmed, leading to a persistent movement for national recognition. This effort culminated in 1920 with its official recognition by a Ministry of Home Affairs ordinance. The Showa era further solidified its standing; the Judo Therapist Law was enacted in 1970, and a major revision in 1989 established it as a fully nationally qualified profession. In the Heisei and Reiwa eras, Judo therapy has continued to adapt. While the number of treatment centers increased, challenges arose concerning the rationalization of medical expenses and ethical standards. Nevertheless, to meet modern needs like an aging society and digitalization, its role has expanded to include preventive medicine, multi-disciplinary collaboration, and the use of DX (digital transformation), showcasing its enduring relevance in contemporary Japanese healthcare. - Effective for diseases without hardness or tenderness. Traditional Chinese medicine (Chinese medicine) is characterized by and places great importance on treating not only illness but also pre-symptomatic symptoms and indefinite symptoms. However, in some cases, induration or tenderness can be detected during palpation, while in others it is more difficult. In cases where this is difficult to detect, treatment is carried out by taking into account the patient's characteristics, the nature of the illness, and the relationship to meridians and acupoints, and by consulting traditional Chinese medicine literature and medical books. The selection of acupoints and their location during this process currently depends on the therapist's expertise and diagnostic ability. One factor that contributes to this dependency on the therapist's skill is the ambiguity of descriptions in classical traditional Chinese medical texts. Acupoints are no exception to this ambiguity. To address this issue, the World Health Organization (WHO) announced in 2008 the international standardization of acupoints and the unification of acupoint locations. This is an important resource for the verification and discussion of traditional Chinese medicine and can be said to have contributed to the medicalization of traditional Chinese medicine. However, it requires anatomical knowledge to confirm the location of acupoints and the skill to explore and access the three-dimensional structure of the human body. Scientific approaches in traditional Chinese medicine have attempted to objectively evaluate the location of acupuncture points and the importance of their stimulation. One example is Ryodoraku. Ryodoraku is characterized by its evaluation using skin current resistance measurements and its use of cathodic direct current needle therapy. Recent research on electroacupuncture therapy has revealed causal and correlational relationships in therapeutic effects. Electroacupuncture therapy is divided into two types: bipolar electroacupuncture, which uses two needles, and cathodic direct current needle therapy, which uses a single needle. Electroacupuncture therapy involves polarity, but different polarities have clear differences in their effects on the human body, and findings regarding their effectiveness have been reported. In electroacupuncture therapy, anodic current needle application has been avoided due to metal corrosion, but metal corrosion is negligible with non-insertive needles. This article explains methods for identifying and evaluating acupuncture points using current resistance, explains cathodic direct current needle therapy, and proposes the importance of therapeutic research utilizing polarity characteristics as a challenge for electroacupuncture therapy. - Effects of Cathode Direct-Current Electroacupuncture Stimulus of the Promixal Anterior Lower Limbs on Heart Rate and Lumbar Blood Flow

Objective: There has been little research on direct-current acupuncture. This study was conducted to verify the effect of acupuncture current intensity and polarity on heart rate (HR) when applying direct-current electroacupuncture (DCEA) to the proximal anterior part of the lower limbs and to clarify the effect of cathodes (-) on lumbar blood flow. Materials and Methods: This study was conducted at Teikyo Heisei University, Chiba, Japan. The subjects were 144 healthy male volunteers (21.7 ± 0.6 years' old). These participants were divided into 1 control, 1 manual acupuncture (MA), and 3 DECEA groups (-25 μA, +75 μA, and -75 μA). Acupuncture was performed on the lower limbs (at ST 36), and HR and intertissue blood flow (ITBF) were observed as the main outcome measures. Results: For intensity, HR was significantly lower in the -75 μA group than in the control (P < 0.001), MA (P < 0.001), and -25 μA (P < 0.01) groups. For polarity, HR was significantly lower in the -75 μA group than in the +75 μA group (P = 0.0028). For blood flow, the volume of HbO2 in the L-4-L-5 region at 1 minute after stimulation was significantly higher in the -75 μA stimulus group than in the MA group (P < 0.05). There was no change in the T-7-T-8 region. Conclusions: A -75 μA stimulus to ST 36 reduced HR and increased ITBF. - 三叉神経領域への100Hz鍼通電が自律神経系と前頭前野の脳血流に及ぼす影響
